About This Service
The ability to travel and move around the community is fundamental to independence, social connection, employment, education and access to healthcare. For many participants with disability, transport is one of the most significant practical barriers to living a full and active life.
Heartful Care Services provides NDIS-funded travel and transport assistance — both accompanying participants on journeys and building participants’ capacity to travel more independently over time. We support participants across a wide range of transport needs, from regular scheduled trips to appointments and activities, to building the skills and confidence to use public transport independently.
Getting from A to B should not be a barrier to living your life. We make sure it is not.
What We Help With
  • Accompanying participants on journeys by car, bus, train and taxi or rideshare
  • Support for participants to attend medical and allied health appointments
  • Transport support for community activities, social events and recreational programs
  • Transport to and from work, education and training
  • Public transport training — planning journeys, purchasing tickets and navigating stops and stations
  • Building confidence and practical skills for independent travel
  • Support for participants using mobility aids, wheelchairs or requiring transfer assistance
  • Assistance with vehicle access and boarding for participants using modified vehicles
  • Coordination of transport logistics for complex or multi-destination trips
  • Emergency transport support where normal transport arrangements break down
  • Travel familiarisation — accompanying participants on new routes until they are comfortable
Our Approach
We approach transport support with a capacity-building mindset wherever the participant’s goals include increasing travel independence. We build skills, confidence, and familiarity with routes and transport systems — not dependency on support workers to accompany every journey.
For participants who require ongoing transport support, we provide reliable, punctual and respectful assistance that ensures you get where you need to go safely and on time. All our workers are trained in safe passenger assistance techniques, including support for participants using wheelchairs, mobility aids or requiring physical assistance to board and alight vehicles.
