Having a safe, suitable and stable home is foundational to every other aspect of a good life. For many people with disability, finding and maintaining appropriate housing is one of the most significant — and most challenging — aspects of daily living.
Heartful Care Services provides NDIS-funded accommodation and tenancy support to help participants find suitable housing, make a successful transition into their new home, and maintain their tenancy with confidence and growing independence.
Whether you are moving from a family home to independent living, transitioning between accommodation arrangements, navigating the private rental market, or working toward specialist disability accommodation, our team provides the practical guidance and hands-on support you need — at every stage of the process.
What We Help With
- Assessing your housing needs and identifying appropriate accommodation options
- Searching for and applying for suitable rental properties in Canberra and ACT
- Supporting you to understand tenancy rights, responsibilities and lease agreements
- Preparing for a move — packing, organising utilities and setting up your new home
- Practical and emotional adjustment support during the transition into a new home
- Liaising with real estate agents, housing providers, social housing agencies and government bodies
- Supporting participants in specialist disability accommodation (SDA) settings
- Establishing and maintaining connections with neighbours and the local community
- Identifying and addressing risks to tenancy sustainability — rent arrears, property maintenance, neighbour concerns
- Coordinating with support coordinators and other providers to ensure housing stability
- Crisis accommodation planning and support where tenancy is at risk
- Building capacity for independent tenancy management over time
Our Approach
Housing support is both practical and deeply personal. We approach every participant’s housing situation with genuine respect for their preferences, their privacy, and their absolute right to choose where and how they live. We are not here to make decisions for you — we are here to give you the information, tools and practical assistance you need to make your own informed choices.
We work collaboratively with your support coordinator, housing providers, and government agencies to remove barriers and advocate on your behalf where needed. No two housing situations are the same, and we tailor our support accordingly.
